Outcomes following nonsurgical treatment for radiographically unstable, severe malar fractures.
There are no studies that specifically address the outcomes following nonsurgical treatment for severe malar fractures. The purpose of this study was to compare the long-term outcomes in patients who underwent nonsurgical treatment for radiographically unstable, severe malar fractures with those who underwent surgical treatment. A retrospective chart review of 20 patients who had undergone nonsurgical treatment for a malar fracture and 20 patients who had undergone surgical treatment was performed. The records were collected for an average of 9.8 years after the nonsurgical treatment and 7.9 years after the surgical treatment. The average time to union was 6.3 months in the nonsurgical group. The average time to union was 12.8 months in the surgical group. Eighteen percent of the nonsurgical patients had delayed union, and 30% had nonunion. Thirty-three percent of the patients who had had the nonsurgical treatment had complications. All of the patients who had had surgical treatment had good or excellent outcomes.
